NDIS Support Across South East Queensland

Finding a good NDIS support worker shouldn't feel like a second job.

But for a lot of families, that's exactly what it becomes. Chasing updates. Following up on gaps. Wondering if the person helping with personal care, daily living, or community access is actually the right person. Feeling like you have to manage the whole thing yourself just to keep it moving. That's not a support problem โ€” that's a provider problem. And it's what Hearts In Action was built to fix. When support is done right โ€” whether it's building independence, developing social skills, or preparing meals together โ€” you feel it. Things settle. Progress happens. You stop bracing for the next problem and start trusting that the people around you actually have it handled.

Hear from Grace and the team about why Hearts In Action exists โ€” and what support looks like when it's built around the participant, not around what's convenient for the provider.

Most NDIS disability support providers deliver a worker. We build a support system.

It's not a small difference. One leaves you hoping things go well. The other is built so they do.

The Matching
  • Matched on personality, routine, communication style, and life goals
  • Not whoever's available โ€” whoever's genuinely right
  • You have full say before anyone starts
The Ongoing Structure
  • Team leader check-ins are built into how every placement works
  • Workers are supported, guided, and never left to sink or swim
  • Issues are caught before they become your problem
The Result
  • You have one conversation to start. After that, it runs.
  • Progress is tracked and visible โ€” not something you have to ask about

Trusted NDIS Support Workers Across South East Queensland

What it feels like when support is finally working the way it should.

01 โ€” From Day One
You'll notice the difference before the first shift is over.

From the first conversation, something feels different. It's not a form. It's not a checklist. It's a real discussion about your life, your goals, and who would actually fit โ€” whether you're living with a physical disability, managing a psychosocial condition, or supporting a family member with autism or an intellectual disability. By the time your support worker walks through the door, they already know you โ€” because we made sure of it.

  • The relief when your worker already knows the morning routine
  • A team leader who calls you โ€” before you have to call them
  • The feeling of not having to explain everything from scratch

That first shift often sets the tone for everything. We take it seriously because families remember it.

02 โ€” As It Settles
Something quietly shifts. You realise you haven't had to chase anything.

Somewhere in the first few weeks, families notice something they didn't expect: the absence of problems. Shifts run. Notes get reviewed. The team leader checks in โ€” with you, not just about you. Progress is visible. And you realise you haven't had to follow anything up. That's not luck. That's the structure working.

  • A week goes by and nothing went wrong
  • You check your phone and there's an update you didn't have to ask for
  • Goals are moving and someone else is tracking them

The quiet is the point. It means everything is handled.

03 โ€” Long Term
Families stay because trust has been earned โ€” and it keeps being earned.

The families who stay with Hearts In Action don't stay out of habit. They stay because week after week, the support is still working. The worker still shows up knowing them. The team is still paying attention. And the goals โ€” the real, meaningful goals โ€” are still moving.

  • Your worker remembers the small things โ€” not just the plan
  • Six months in and the standard hasn't dropped
  • You stop thinking about support because it just works

That's the goal: support that earns your trust, then keeps it.

Here When You're Ready

Starting again with a new provider takes something. We know that.

If previous support let you down โ€” or let someone you love down โ€” the idea of trying again can feel like too much. We hear that from a lot of families. Whether your family member lives with a physical disability, autism, a psychosocial condition, an intellectual disability, or an acquired brain injury โ€” the cycle of hope, disappointment, and starting over is the same. We don't take lightly that you might be in that place right now.

Here's what we can honestly say: we built Hearts In Action because that experience is too common and too unnecessary. We take on fewer participants than we could, because doing this right matters more than doing it at scale.

If we're not the right fit, we'll tell you. And if we are โ€” we'll show you, consistently, every week. The decision always stays with you.

NDIS Disability Support Workers SEQ ยท Frequently Asked Questions

NDIS support worker questions families ask before booking

Straight answers. No sales language.

01 How do you work with families and support coordinators?

We communicate directly and proactively โ€” families and coordinators are kept informed without having to ask. Your participant's dedicated team leader is the single point of contact for both. We stay aligned on goals, flag changes early, and send documented updates. Coordinators get proactive reporting, not last-minute summaries at plan review time.

02 What does getting started look like?

A phone call. We listen to your situation, understand what you need, and think about fit. If we can help, we walk you through what comes next clearly. Nothing starts until everything makes sense to you.

03 How are things kept accountable?

Every shift is documented in Astalty, our professional record-keeping system. Your team leader reviews these notes daily โ€” not weekly, not at review time. Every participant gets a formal 60-day check-in to make sure goals are on track and the match is still right. There's no system that relies on you following things up โ€” the structure is built so we catch things before they become problems.

04 What happens if something goes wrong?

We deal with it โ€” not you. Your team leader is the first point of contact for any issue โ€” and they're reachable by phone, not just email. We address it, communicate with you clearly, and make sure support gets back on track. If a worker is sick or unavailable, a pre-approved backup from your existing team steps in โ€” no gaps, no strangers.

05 Can I use my NDIS plan with Hearts In Action?

Yes. We work with self-managed, plan-managed, and NDIA-managed participants across South East Queensland โ€” Gold Coast, Logan, Ipswich, and Brisbane. If you're unsure how your funding works or whether your plan covers the support you need, just ask on the call โ€” we'll explain it clearly and honestly.

06 How long does it take to get started?

After the initial conversation, we take time to match properly rather than quickly. How long depends on your needs and location, but we'll give you a realistic timeline upfront โ€” and we won't rush a match just to fill a slot.

07 I'm a support coordinator โ€” how do you keep me in the loop?

Your participant's team leader is your direct contact โ€” not a generic inbox or receptionist. You get proactive updates, documented shift notes via Astalty, and early flags on any changes in goals, behaviour, or circumstances. At plan review time, you'll have real data, not scrambled summaries. We don't wait for you to chase us โ€” that's the whole point.

08 What does an NDIS support worker actually do?

An NDIS support worker helps participants with the practical and personal parts of daily life โ€” things like personal care, meal preparation, community access, transport to appointments, domestic assistance, and building life skills. At Hearts In Action, every support worker is matched to the participant's personality, routine, and goals. They're not just helping with tasks โ€” they're working toward real, measurable progress in the areas that matter most to you.

09 How do I find a good NDIS disability support worker?

Start by looking for a registered NDIS provider โ€” registration means they've been independently audited and meet quality standards. Then look beyond the website. Ask how they match support workers to participants. Ask who oversees the placement after it starts. Ask what happens when something goes wrong. A good provider will have clear answers to all of these โ€” because they've actually built the systems to back them up.

10 What's the difference between a registered and unregistered NDIS provider?

A registered NDIS provider has been assessed and audited by an independent body to meet NDIS Practice Standards. They're held accountable through regular audits โ€” for Hearts In Action, that's every 18 months. Unregistered providers have no external oversight. If you're NDIA-managed, you can only use registered providers. If you're self-managed or plan-managed, you can use either โ€” but registration is a strong quality signal that the provider takes accountability seriously.

11 Can I choose my own NDIS support worker?

Yes โ€” and at Hearts In Action, we insist on it. We don't just assign whoever's available. We take time to understand your personality, your routine, and what matters to you, then match you with a support worker we genuinely believe is the right fit. You meet them before anything starts, and you always have the final say.

12 What types of disabilities do you support?

We support NDIS participants living with a wide range of disabilities across South East Queensland โ€” including autism and ASD, psychosocial and mental health conditions, intellectual disabilities, physical disabilities, and acquired brain injuries. Our support workers are matched based on experience, personality, and the specific needs of each participant. If you're unsure whether we can help with your situation, just ask on the call โ€” we'll be straight with you.

13 What areas do you cover in South East Queensland?
