Meet Grace
Family. Personal. Connected.
Hearts In Action is a small, family-owned NDIS provider, and that shapes the way we support people. Grace shares why staying personal, accessible and genuinely connected to participants matters to us and why we never want support to feel like just another service.
We Work Alongside Your Allied Health Team
Connected. Informed. Consistent.
With your consent, we work alongside your allied health professionals including occupational therapists, psychologists, physiotherapists, speech pathologists and behaviour support practitioners so your day-to-day support reflects the strategies and goals developed with you.
If you have a Positive Behaviour Support Plan, we take the time to read and understand it, helping ensure relevant strategies are carried through consistently in your everyday support.
It means better communication across your support team, while you remain at the centre of every decision.
Tell Us What’s Working And What Isn’t
Open. Heard. Responsive.
Your feedback matters. If something is working well, we want to know and if something needs to change, we want to hear that too.
You should feel comfortable speaking openly about your support. We listen, respond and work with you to make changes when needed.
Your Goals Set The Direction
Your Goals. Your Pace.
Your NDIS goals belong to you. You might know exactly what you want to work towards, or you may still be figuring that out.
Your support worker is there to help you move towards what matters to you, while respecting your pace, preferences and decisions. As your life changes, your support can change with it.
Consistency Builds Trust
Familiar. Predictable. Personal.
Having support in your home or out in the community is personal. That’s why we place such a strong focus on consistent support worker relationships.
Over time, your worker gets to know your routines, preferences, communication style and when you want support and when you’d rather do things yourself. That familiarity can make support feel more comfortable and predictable, without ever taking your choices for granted.
Meet Our Team
Skilled. Dependable. Genuine.

We hire for attitude, reliability and professionalism, then back our team with strong systems, leadership and dedicated team support.
The aim is simple: less time worrying about support, and more time getting on with your life.
