The People on Your Team
Everyone on this team is here because they care. The people we support are at the centre of everything we do.
Matt worked as a support worker and saw it first-hand. Participants and families were carrying stress that should have been handled by providers. He didnβt want to wait, so he started Hearts In Action. Heβs now studying a Bachelor of Psychology to build better support systems.
Before Hearts In Action, Grace led a childcare centre. Thatβs where she saw how much it matters to feel safe, seen, and heard. It doesnβt only apply to children. Everyone should feel that way, and thatβs the standard we expect of our team.
Antaylia grew up in a SIL home. She knows what good support feels like, and what poor support is like to live with. She saw workers come and go, and which ones made her feel comfortable at home. Thatβs why participants feel comfortable talking to her. She understands it from lived experience.

Before You Book
Questions Before You Get Started
Support workers donβt move on often. If it does happen, youβre not starting over. Your worker is part of your team, not the whole thing.
You choose who comes next. We donβt send whoeverβs available. Once youβve chosen, we brief them on how your support works, what matters to you, and what doesnβt, so youβre not re-explaining everything.
Matt, Grace, and Antaylia already know your situation, and that knowledge stays with us, whether your worker changes or not.
Every support worker meets with Grace before they start, completing NDIS screening, Blue Card checks, past employment reference checks, and our internal training.
Once support begins, your team leader reads every shift note the same day and stays across whatβs happening, so any issues are picked up early.
If at any point something doesnβt feel right, just let us know. Weβll have a conversation about whatβs not working and see if it can be resolved in a way youβre comfortable with.
If not, weβll introduce someone new for you to meet. Weβll keep working with you until you feel confident and comfortable with your support worker.
Support is personal, and we take that seriously. Your comfort comes first.
Weβve listened closely to the NDIS community and the challenges people have faced. Thatβs why weβve built our system around clear communication, consistent service, and accountability at every level.
Youβll be able to contact your team leader directly, which makes difficult conversations easier and removes the stress of having to manage things yourself. It means you can focus more on your life, not the coordination and how your support is performing.
Behind the scenes, we have systems in place to make sure everything runs as it should. Records are kept in Astalty, a purpose-built NDIS platform, which helps reduce human error and ensures accurate, consistent record keeping.
In most workplaces, thereβs a manager on-site who can see how things are going day to day and step in early if something feels off.
Support work is different. Your worker is in your home, and their employer isnβt there to observe each shift.
If youβre working with an independent support worker, thereβs no built-in oversight. That means concerns rely on you to notice, raise, and manage.
Your team leader provides that extra layer of support.
They review shift notes daily, check in regularly with your worker, and stay in contact with you. If something starts to change, like notes not aligning with your goals or a workerβs attendance slipping, they can step in early.
You can contact your team leader anytime. Theyβll also check in with you every 60 days to make sure things are working well and address any concerns.
