1. Initial Connection: We begin with a phone conversation to understand your immediate needs.
2. Face-to-Face Consultation: We meet in person to build rapport and discuss specific requirements.
3. Support Planning: We develop a robust, personalized support plan tailored to your goals.
4. Digital Sign-Off: We return for a guided digital signing session to ensure you are fully informed before support begins.
We utilize Astalty as our internal practice management framework to ensure 100% NDIS legislative compliance and precise documentation. This system generates goal-mapped case notes and audit-ready invoicing, ensuring that all service delivery data is recorded accurately and professionally.
We maintain high-quality case notes designed to proactively identify and flag emerging concerns before they escalate into critical issues. This provides the ability to oversee the whole operation necessary to maintain participant safety and service continuity.
We conduct proactive check-ins with participants every 60 days to evaluate service satisfaction and ongoing goal alignment. This recurring review cycle ensures that support remains relevant to the participant's evolving needs and allows for the early identification of any required plan adjustments or resource reallocations.
