We assess whether we can genuinely deliver what's needed. If we can help, we match a support worker. If we can't, we tell you straight โ€” and point you somewhere useful.

The participant and family have the final say.

Yes. We're accepting new participants across Gold Coast, Logan, Ipswich, and Brisbane. We keep our numbers deliberately manageable โ€” so availability can change. If you're considering a referral, the best approach is to call and check. We'll give you a straight answer immediately.

02 How do I refer a participant to Hearts In Action?

Call us or book a provider call. We'll discuss the participant's needs, disability type, goals, and what matters to them and their family. If we can help, we explain the matching process and timeline. If we can't, we'll tell you honestly and suggest alternatives where we can.

03 What reporting do I receive and how often?

Every shift is documented in Astalty โ€” our professional NDIS record-keeping platform. Your participant's team leader reviews notes daily and flags anything that needs attention. You receive proactive updates without having to ask. At plan review time, you get compiled progress data, goal tracking, and documented outcomes โ€” not a last-minute summary.

04 How quickly can you start after a referral?

It depends on the participant's needs and our current capacity, but we'll give you a realistic timeline upfront. We prioritise matching properly over matching quickly โ€” rushing a placement to fill a slot isn't something we do. Most referrals move from initial conversation to placement within 1โ€“2 weeks.

05 What happens if the match doesn't work?

We address it directly. If a participant or family isn't happy with the match, we reassess and propose a new worker. The team leader manages this process โ€” you don't have to. We'd rather get the match right than pretend a problem doesn't exist.

06 Can you support participants with complex or high needs?

It depends on the specific needs. We support participants across a range of disability types โ€” autism, psychosocial conditions, intellectual disabilities, physical disabilities, and acquired brain injuries. For complex support needs, we'll have an honest conversation about whether our team has the right experience and capacity. We won't take on a placement we can't deliver well.

07 What makes you different from other NDIS support providers in SEQ?

Three things. First, every participant gets a dedicated team leader who reviews shift notes daily and conducts formal 60-day check-ins โ€” not just a worker placed and forgotten. Second, we have documented continuity plans so support doesn't collapse when a worker is unavailable.
