NDIS Support Workers · Brisbane
The Same Brisbane Support Worker, Every Single Week
Looking for a disability support worker Brisbane families can count on? Hearts In Action is a registered NDIS provider that finds the very best workers across the city, northside and southside, for personal care, community access and capacity building. Self-managed, plan-managed and NDIA-managed plans are all welcome.
Support That Starts With Listening
Good support starts with listening. Our Brisbane support workers take the time to learn what matters to you before anything gets locked into a roster, because genuine care never looks the same twice.
Maybe that's help with the morning routine, company at an appointment across town, or a walk along the river. Maybe it's steady encouragement while you work on something new. Whatever it is, we ask what you actually want first, then shape the support to match, and your goals lead every single visit.
Every participant has their own idea of a good day, and our job is to make it happen. One participant was chasing a proper adrenaline hit, so his NDIS support worker took him full-motion VR racing in Brisbane: strapped in, engine roaring, all the thrill without the real-world risk. For someone else it might be a quiet coffee, a kick of the footy at the park, or the same weekly shop done together, because familiar routines count for just as much as big days out.
What Your Brisbane Support Worker Can Actually Do for You
A good support worker makes day-to-day life easier to manage, not just something to invoice. This is the everyday work of a support worker Brisbane families rely on:
Reliable transport and company for appointments, therapy and errands, anywhere in the city, minus the stress of getting there
Personal care and morning or evening routines handled by the same familiar face, not a rotating roster
Getting out and about, whether that's a regular group, a new hobby, or catching up with mates across town
Practical, hands-on backing while you work on new skills, at whatever pace suits you
A dependable backup when the week gets hectic or your usual support falls through
The Fit Is Your Call
We introduce you to workers we think will suit you, then you make the decision. And if the match isn't right down the track, just say the word and we'll handle the change without any awkwardness.
The Same Faces, Week After Week
You shouldn't have to re-explain your routine to a stranger every visit. We keep the same Brisbane support worker, or a small consistent team, on your plan so the support actually builds on itself.
You Set the Direction
It's your plan, your goals and your call. We check in regularly and adjust whenever life changes, because support that stops fitting isn't really support.
Straight Through to Your Team
Ring 07 2812 2406 whether it's a schedule change, a funding question, or something that needs sorting today. Whoever picks up already knows your plan.
Reliable Support, Not Just Promises
Anyone can promise good support on a website. These four standards are what you get from a support worker Brisbane families keep, from the first visit onward.

Self-Managed
You hold the funds and pay providers yourself, which gives you the widest choice, registered or unregistered. As a registered provider, we fit right in.
Plan-Managed
Your plan manager handles the invoices and claims while you keep full choice of providers, registered or not. You pick the support, they do the paperwork.
NDIA-Managed
The NDIA pays registered providers directly through the myplace portal. Because Hearts In Action is NDIS-registered, NDIA-managed participants can absolutely work with us.
However Your Plan Is Managed, You're Covered
Every support rate is capped by the National Disability Insurance Scheme Pricing Schedule 2026-27. What changes from plan to plan is how yours is managed, so here's what each of the three types means when you work with a registered provider like us.
Unsure which category you're in, or what your plan will actually fund? Give us a call on 07 2812 2406 and we'll walk through it together, no strings attached. Or see how long your funding will last with our free calculator.
Brisbane Suburbs We Cover
Brisbane splits at the river, northside and southside, and our NDIS support workers cover both sides:
Brisbane Northside
Brisbane Southside
Can't see your suburb? We service all of Brisbane, and that goes for every one of the support workers Brisbane families meet. Just ask.
Closer to Logan, Ipswich or the Gold Coast? We have dedicated local pages for our NDIS support workers in Logan, support workers in Ipswich and support workers on the Gold Coast.
Weighing up organisations rather than individual workers? See how to choose an NDIS provider in Brisbane.
Assistance with Daily Life
Personal care, meals, and the day-to-day tasks at home, covered by a consistent worker you know.
Social & Community Participation
Support to be out in the community: appointments, events, groups, or simply places that matter to you.
Capacity Building Supports
Skills and confidence built gradually, so independence grows at a pace that works for you.
NDIS Support Categories We Deliver in Brisbane
Your plan groups funding into standard NDIS categories. Your Brisbane support worker works across all three.

NDIS Support Worker Brisbane FAQ
How much does an NDIS support worker cost in Brisbane?
Support worker prices are capped by the National Disability Insurance Scheme Pricing Schedule 2026-27, the scheme's official price list, and your plan funds them. The capped rate depends on the support type, the day of the week and the time of day. You can look up the current capped rate for any support in our free NDIS price guide, and we'll explain exactly what applies to your plan on the phone.
What's the difference between an NDIS support worker and a disability support worker?
The terms overlap almost completely. Strictly speaking, an NDIS support worker delivers supports funded through an NDIS plan and works to its goals and categories, while disability support worker is the broader job title. Day to day, people use both to mean the same thing.
Can I choose my own support worker?
Yes, always. You meet the workers we suggest, you make the call, and if the fit changes later you can ask for someone new whenever you like.
Can you work with NDIA-managed participants?
Yes. As a registered provider we support NDIA-managed participants, along with self-managed and plan-managed plans.
What areas of Brisbane do you service?
We cover the whole Brisbane region, both sides of the river, from Sandgate and Chermside on the northside down to Sunnybank and Forest Lake on the southside. If you're in Brisbane, we can support you.
How quickly can a support worker start?
Usually very quickly. Exact timing depends on your needs and worker availability near you, and we'll give you a straight answer with a real timeframe on the first call.
What checks do your support workers have?
Every worker carries a current NDIS Worker Screening Check and Blue Card, along with First Aid/CPR certification, infection control training, and relevant qualifications such as a Certificate III in Disability Support.
Do you provide short-notice or respite support?
We can often help at short notice, depending on availability. The fastest way to find out is to call 07 2812 2406.
Do you service all of Brisbane?
Yes. We cover the whole Brisbane region, northside and southside, from the inner city out to the surrounding suburbs.
What if my current provider isn't working out?
Switching providers is more common than you'd think, and it doesn't have to be messy. We coordinate the changeover so there's no gap in your support. Our Brisbane provider page covers how switching works.
Is there a 2 hour minimum for NDIS supports?
Usually yes, but it doesn't come from providers or the NDIS. The SCHADS Award, the industry award that covers support workers, sets minimum engagement periods, generally two hours for this kind of work. The rule exists so workers are treated fairly and have financial security: without reliable income, good support workers leave the industry, and consistent support depends on keeping them in it. We'll always be upfront about how minimum shifts apply to your supports before anything is signed.
How do I get a support worker through the NDIS?
If you already have an NDIS plan with Core funding, it's simpler than most people expect: no referral needed. You choose a provider, meet the worker they suggest, and support starts once you're happy with the fit. Call 07 2812 2406 and we'll introduce you to a disability support worker Brisbane families already trust, before anything is signed. If you don't have an NDIS plan yet, you'd first check your eligibility and apply through the NDIA, and we're happy to point you in the right direction on the same call.
