Families Supported in Logan

Where it started

An overview of our system at Hearts In Action.

Participants in Logan have given us a lot of feedback from their experience with other providers about what's going wrong. We've used that to build a system that delivers a better service.

The usual problems

Six things families tell us broke down before β€” and what we built differently.

If any of these sound familiar, you're not alone.

Where it breaks

What families keep telling us.

The support worker keeps changing.Whether it's a company sending whoever's available, or an independent who's left or gone on holidays. It's up to you to work it out.

01

You get a consistent team of two.If someone's away, or on a rare occasion someone leaves, you're not starting from scratch. Antaylia, Matt, and Grace are part of your team. They get things back on track, so you don't have to worry about it.

02

You need support, but also have to manage it.A worker is placed by a company with no oversight. Or you've taken on an independent, hoping everything works out. When it doesn't, you have to manage it.

02

Need help? Call Antaylia. As your team leader, she implements the changes you ask for, manages any performance issues, and keeps support on track.

03

They cancel an hour before the shift. One text message, and now you're scrambling to reorganise the whole day.

03

We call you.We explain what's happened. And how we can help minimise the impact on your day.

04

You are now the manager. So schedules, training gaps, and difficult conversations all land on you.

04

Our team leader takes care of it.So rosters, worker performance, and feedback are all managed for you.

05

The numbers don't add up.You're left with charges that don't line up with the shifts you actually received.

05

All of our invoices come directly from Astalty.A leading NDIS software provider, and are based on actual clock-in and clock-out times.

06

It can be stressful speaking up.You've asked previous workers to make changes, and you can feel the tension. All you want is support that suits you.

06

We're a professional team. All of our support workers understand what person-centred support actually means. You can speak to them or Antaylia, whichever feels more comfortable.

The People Behind Your Support

The People Behind Your Logan Support

Matt, Grace, and Antaylia built the structure, together with our Logan support workers. They're still the ones who answer for how it works.

Matt - NDIS Provider Director Hearts In Action Logan
Matt
Director

Matt set up the structure behind Hearts In Action. By designing every system around what Logan families told us was missing. So the accountability is built in, not bolted on.

Grace - Director interviewing Logan NDIS support workers
Grace
Director

Grace decides who gets onto the team. By meeting every support worker face to face and putting them through internal training before they meet anyone. So if they wouldn't clear her own standard, they don't start.

Antaylia - Logan Team Leader overseeing disability support
Antaylia
Team Leader

Antaylia is the person between you and your worker. By reviewing every shift note the day it's written and picking up when you call. So anything that needs attention gets caught before it becomes a problem.

Our team. Some cover the Logan area. All deliver quality support.

It's your choice who supports you. Every support worker is NDIS-screened, Blue Card verified, first aid and CPR trained.

Before You Book

The Questions Logan Families Ask Us Most

If your support worker leaves, your support doesn't reset. They're one part of your team, and the rest of your team already knows your routine, preferences, and what matters to you.

Nothing gets lost. We carefully find someone who fits, and you're part of that decision. You're guided through the process from start to finish.

Before any worker meets a family, Grace interviews them directly. They go through NDIS screening, full police background checks via Blue Card, and internal training before their first shift.

After they start, your team leader checks every shift note that day. They stay in close contact with you and your worker, so there's daily oversight on your support.

Our role is to suggest the right people, but you always have the final say. You meet your worker before starting, and if it's not a good fit, we'll introduce someone else.

If you decide later that it's not working, you don't need to justify it. Just tell your team leader, and we'll take care of the change.

Most issues with other providers come down to communication, consistency, and accountability. That's what we've built our approach around.

You have one team leader who knows you and is easy to reach. Your worker stays consistent, and every shift is reviewed the same day.

If something isn't right, you call your team leader and it gets handled. Many families come to us after a poor experience, and we don't take that trust lightly.

Your team leader checks every shift note the day it's written and stays connected with you and your worker. That way, anything that needs attention is picked up quickly.

If something needs adjusting, you can speak to your team leader straight away. Every 60 days, you'll review everything together.
