For Families Who've Been Holding It All Together

You've Already Done the Hardest Part.

You've been the advocate, the scheduler, the backup plan. That was never your job. It's ours. Meet a worker who learns the routine โ€” you decide if they're right. Weekly oversight and daily shift note reviews mean you can finally step back.

Your Life. Your Routine. Your Call.

A support worker who remembers how you like things done โ€” not someone reading your file for the first time. You choose who's right, and the same person keeps showing up. If they're ever away, your backup already knows you.

NDIS Support Across South East Queensland

Finding a good NDIS support worker shouldn't feel like a second job.

For too many families, that's exactly what it becomes. Chasing updates. Wondering if the person helping with personal care, daily living, or community access is actually the right fit. You become the fallback every time something falls through.

That's a provider problem. It's what Hearts In Action was built to fix. When support is done right, you feel it. Things settle. Progress happens.

Hear from Grace and the team about why Hearts In Action exists โ€” and what support looks like when it's built around the participant, not around what's convenient for the provider.

Most providers hope things go well. We built a system so they do.

It's not a small difference. One leaves you chasing, worrying, and filling gaps yourself. The other lets you step back because someone else is actually watching.

If you've been managing all of this yourself, you already know the difference. Here's what it looks like side by side.

Most Providers
A transaction
Hearts In Action
A built system
Worker arrives, then silence. You're on your own.
You meet your support worker first. You decide if they're right.
No one to call when you need to discuss how support is going.
A team leader who knows your situation and picks up the phone.
You do all the coordination. Scheduling, chasing, checking.
Every shift reviewed. Goals tracked. Nothing falls through.
Cancellations get charged to you, even when the provider could fill the shift.
If we can fill a cancelled shift, you're not charged.
Workers leave. You start over. Again and again.
Backup is pre-arranged. Your support never stops.
How It Works

Built so support stays consistent โ€” and you stay in control.

Whether you're the person receiving support or the family behind them โ€” this is how it works once you say yes.

Step One

You Choose Your Worker

We listen to what matters to you
We suggest someone we think could be a great fit
You meet them first โ€” no commitment
We start when you're ready
Step Two

A Team That Stays Across It

A dedicated team leader oversees your support
Every shift note reviewed daily
One message to the team leader and we handle the rest
Scheduled 60-day check-ins with you, your family, and your support coordinator
Step Three

Support That Doesn't Need Managing

The system runs. The team stays across it.
Progress tracked and reviewed without asking
Support runs in the background. Your life stays in the foreground.
We built this so participants get consistency and families get peace of mind. That's our system โ€” it's how our support is designed to work.

What it feels like when support is finally working.

Families stay because six months later, they're still getting what we promised on day one. Your support worker still remembers the routine, the preferences, the small things. The oversight hasn't faded.

One Dedicated Point of Contact Per Participant
01 โ€” From Day One

You'll notice the difference before the first shift is over.

  • The relief when your worker already knows the morning routine because they were properly briefed
  • Someone calls you before you have to call them โ€” so you stop bracing for problems
  • The feeling of not having to repeat your whole story because someone actually listened the first time
02 โ€” As It Settles

A few weeks in, you realise you've stopped checking up on everything.

  • A full week passes and nothing fell through the cracks
  • You check your phone and there's an update you didn't have to ask for
  • Goals are moving forward and someone else is keeping track
03 โ€” Long Term

Months later, the standard is exactly where it was on day one.

  • Your worker remembers the small things, not just what's written in the plan
  • Six months in and you're still getting the same standard, not a watered-down version
  • You stop thinking about support altogether because it just works

Here When You're Ready

Starting again with a new provider takes something. We know that.

If previous support let you down, the idea of trying again can feel like too much. We hear that from a lot of families.

We take on fewer participants than we could, because doing this right matters more than doing it at scale. If we're not the right fit, we'll tell you. If we are, we'll show you every week. The decision always stays with you.

We build the team. You meet them first and choose the support worker.

For Support Coordinators

A registered NDIS provider you can refer to without second-guessing

You've referred participants to providers who started strong and faded. We built Hearts In Action so that doesn't happen. We run 60-day check-ins with the participant, their family, and with you.

Proactive Updates
You'll hear from us before you have to ask. Your participant has a dedicated team leader as their point of contact. When check-ins come around, you'll have real data to work with.
Documented Shift Notes
Every shift is logged in Astalty and reviewed the same day โ€” creating a live thread between participant, worker, and leadership that keeps everyone aligned between check-ins.
Registered & Audited
NDIS registered, ISO 9001 certified, audited every 18 months. We track clock-in/out times, invoices, shift notes, and every worker credential so nothing lapses and nothing gets missed.
Continuity Guaranteed
When a worker is sick or on leave, a pre-approved backup from the existing team steps in. No gaps, no scrambling, no strangers โ€” just a familiar face keeping the routine intact.
Refer with confidence. We'll earn it.

NDIS Disability Support Workers SEQ ยท Frequently Asked Questions

NDIS support worker questions families ask before booking a chat

Straight answers to real questions.

01 Can I choose my own NDIS support worker?

Absolutely โ€” and we insist on it. Through conversation, we get to know what matters to you โ€” your routine, your preferences, what a good day looks like. From there, we build a shortlist of workers we believe are the right fit. You meet them before anything starts, and you always have the final say.

02 Who do I contact about my support?

You and your family have direct contact with a team leader assigned to your support. They know your situation, your goals, and your team. If a worker is unavailable, a pre-approved backup from your existing team steps in.

Small challenges get addressed early because your team leader is already across your support โ€” not hearing about it for the first time.

03 How are things kept accountable?

Every shift is documented in Astalty and reviewed the same day. Every participant has a formal 60-day check-in to make sure goals are progressing and the fit is still right.

When something comes up, you reach your team leader directly โ€” the person who already knows your situation and can sort it out.

04 What does getting started look like?

A phone call. We listen, understand what you need, and talk through what good support looks like for you. From there, we arrange a meet and greet with workers we think are a good fit. You and your family choose who starts โ€” nothing moves forward until you're comfortable.

05 How long does it take to get started?

Generally within a week. After the initial conversation, we build a shortlist of workers who fit your needs โ€” then you choose who to meet. We won't rush the process just to fill a slot.

06 Can I use my NDIS plan with Hearts In Action?

Yes. We work with self-managed, plan-managed, and NDIA-managed participants across South East Queensland.

We practise transparent billing. If a worker cancels and we can fill that time, you're not charged.

07 How do you work with families and support coordinators?

Support coordinators know what it's like to refer a participant and then chase for updates. Families know what it's like to feel out of the loop. Neither happens here.

Every participant has one named team leader who handles communication with both families and support coordinators. We flag changes early and send documented updates.

08 I'm a support coordinator โ€” how do you keep me in the loop?

You'll have the contact details of your participant's team leader. That means proactive updates, documented notes via Astalty, and early flags on any changes. We update support coordinators every 60 days with real data you can use at review time.

09 What's the difference between a registered and unregistered NDIS provider?

A registered provider has been assessed and audited to meet NDIS Practice Standards โ€” for Hearts In Action, that's every 18 months. Unregistered providers have no external oversight. If you're NDIA-managed, you can only use registered providers.

With a registered provider like HIA, you get a structured team โ€” team leaders, documented shift notes, 60-day check-ins, and credentialed workers. With unregistered or independent providers, that structure is optional.

10 What does an NDIS support worker actually do?

An NDIS support worker helps with the practical and personal parts of daily life โ€” personal care, meal preparation, community access, transport, domestic assistance, and building life skills. At Hearts In Action, you choose the support worker. They work toward real, measurable progress in the areas that matter most to you.

11 How do I find a good NDIS disability support worker?

Start with a registered NDIS provider โ€” registration means independent audits and quality standards. Then ask the hard questions: are workers fully credentialed? Who do you call if you'd like to discuss your support and how your support worker is performing? Do you get the final say on who supports you?

A good provider will have clear answers to all of these, because they've built the systems to back them up.

12 What types of disabilities do you support?

We support participants living with a wide range of disabilities across SEQ โ€” including ASD, multiple sclerosis, fibromyalgia, major depression, GAD, intellectual disabilities, psychosocial disabilities, cerebral palsy, and acquired brain injuries. We suggest workers based on experience and specific needs โ€” you decide.

13 What areas do you cover in South East Queensland?

Hearts In Action provides NDIS disability support workers across South East Queensland โ€” covering the Gold Coast, Logan, Ipswich, and Brisbane regions and surrounding suburbs. Each area has a dedicated page: Gold Coast, Logan, Ipswich, Brisbane.

Still have a question that's not here? Just ask on the call. That's what it's for.
