We started Hearts In Action because too many families were being let down by providers who talk about care but don't build the systems to deliver it.

The NDIS space is full of providers who say the right things. Care. Independence. Empowerment. But when you look behind the words, there's often no structure holding it together. Workers are placed and forgotten. Nobody reads the shift notes — if there even are any. Nobody checks in with the participant. And when something goes wrong, it's the family who has to fix it.

Matt and Grace started Hearts In Action in 2024 because they'd seen that cycle up close — and they refused to accept it was just how things worked. The idea was simple: what if an NDIS provider actually built the systems to back up its promises? A team leader who reads every shift note daily. Formal 60-day check-ins with every participant. Workers who are matched on personality and goals, not just availability. Real backup plans so support doesn't collapse when someone's sick.

That's what Hearts In Action is. Not a marketplace. Not a roster service. A managed team of disability support workers across South East Queensland, held together by structure, accountability, and people who genuinely give a damn.

Matt co-founded Hearts In Action because he believed NDIS disability support could be done properly — with real structure, real oversight, and standards that don't fade after the first week. He's the person behind the systems: putting in place daily shift note reviews, the 60-day check-ins, the matching process, the continuity plans. He built these because he saw what happens when they don't exist — and he wasn't willing to run a provider without them. Matt is direct, detail-oriented, and personally invested in every placement running the way it should.

Grace is the reason the team is what it is. She has an instinct for people — not just their qualifications, but who they are, how they show up, and whether they genuinely belong in this work. She co-founded Hearts In Action with the conviction that families deserve support workers who are exceptional, not just adequate. Grace oversees recruitment, onboarding, and the culture of the team. If someone's on this team, it's because Grace believed in them — and she doesn't get that wrong.

She supports workers when they need guidance and holds them accountable when the standard needs reinforcing. When a family calls with a concern, Antaylia is the person who answers — and the person who fixes it. She doesn't manage from a desk. She manages from the front line, which is exactly where a team leader should be.

01 Who runs Hearts In Action?

Matt and Grace co-founded Hearts In Action in 2024. Matt built the operational systems — the matching process, shift note reviews, 60-day check-ins, and continuity plans. Grace leads recruitment and team culture, ensuring every support worker meets the standard families deserve. Together, they're involved in every placement and every decision that affects participant support.

02 What does the team leader actually do?

Antaylia is the team leader and the operational backbone of every placement. She reads shift notes daily, conducts regular check-ins with participants and families, supports workers when they need guidance, and is available by phone when something needs attention. She's the person between you and the system — and she takes both sides seriously.

03 How do you choose your support workers?

Grace leads recruitment with a specific focus: finding people who are genuinely excellent at this work, not just qualified on paper. We look for real-life skills, emotional intelligence, reliability, and the kind of character that families notice immediately. Every worker is interviewed, reference-checked, screened, and observed before they join the team.

04 How many support workers do you have?

We currently have around 10 support workers across South East Queensland. We keep the team deliberately small so we can maintain genuine oversight, real relationships, and consistent quality. We'd rather grow slowly and properly than scale fast and lose what makes this work.

05 Are your support workers employees or contractors?

Our support workers operate within a professionally managed team structure — with a team leader, daily oversight, documented shift notes, and formal check-ins. This isn't a marketplace model where independent workers operate alone. Every worker has leadership, support, and accountability behind them.

06 What training and qualifications do your workers have?

All workers hold current NDIS Worker Screening clearance, First Aid certification, and relevant disability support qualifications. Beyond that, they receive ongoing guidance from the team leader and operate within Hearts In Action's systems — including Astalty for documentation, structured onboarding, and regular performance reviews. Training doesn't stop after induction.

07 Can I meet my support worker before they start?

Yes — always. We don't start support until you've met the proposed worker and given your approval. You'll know who's coming, why we matched them to you, and what to expect. If the match doesn't feel right, we find someone who does. You always have the final say.
