The NDIS space is full of providers who say the right things. Care. Independence. Empowerment. But when you look behind the words, there's often no structure holding it together. Workers are placed and forgotten. Nobody reads the shift notes — if there even are any. Nobody checks in with the participant. And when something goes wrong, it's the family who has to fix it.
Matt and Grace started Hearts In Action in 2024 because they'd seen that cycle up close — and they refused to accept it was just how things worked. The idea was simple: what if an NDIS provider actually built the systems to back up its promises? A team leader who reads every shift note daily. Formal 60-day check-ins with every participant. Workers who are matched on personality and goals, not just availability. Real backup plans so support doesn't collapse when someone's sick.
That's what Hearts In Action is. Not a marketplace. Not a roster service. A managed team of disability support workers across South East Queensland, held together by structure, accountability, and people who genuinely give a damn.
